Omni distorts on monitor output

Hi,

Ive had this problem for a while but have just been using workarounds... quite often there is distortion added to my signal at the monitor section of the omni. if i bypass the monitor section and go direct to outputs 1 & 2 (or 3 & 4 or any others) the signal is clean. It sounds kinda like a blown speaker. When it happens the distortion is clearly audible even at very low levels. its very weird and seems to happen only sometimes and i have absolutely no idea what triggers it..

The system is an HD2 pt 10.3 and the Omni - i upgraded the firmware a couple of months back but that had no affect on the problem

any ideas or suggestions greatly appreciated
