.midnam file needed for E-mu E4XT Utra emulator

I am using Protools LE 8 on a mac pro (OSX).
I have an EMU 4XT sampler.
I need to create a midnam file so I can change patches on my 4XT from Protools. I had a file (which I edited) that worked but it was lost during an upgrade.

The problem is now I can't find or create a file that will load for the 4XT in Protools. When trying to load the .midnam file in Protools I get the message:

'Pro Tools could not find appropriate patch names in the currently selected patch name document because either manufacturer/model information or instrument mode do not match.'

If I can get any file to load for the 4XT, I can edit the xml file for my patches.
