Sound Designer II File Format ?

I need the technical specifications of Sound Designer II File Format.

The DigiDesign WWW server tells that this document can be downloaded at:
ftp://ftp.digidesign.com/pub/support...s/SDIIForm.hqx

Unfortunately, this link is broken (because the FTP directory faxdocs is empty)

I saw that this document can be obtained by fax from the US, using its reference number: 1090. But I am located in France and it does NOT seem to work form here...

I would be glad if you could tell me how to obtain these specifications..., for example by email: [email protected]
