Default No return signal for hardware inserts

I'm having an issue where I can't get the return signal to come back into pro tools when I engage a hardware insert. The hardware is getting the output signal though. This all started when I added a switch craft patch bay; there wasn't a problem before.

I have tested the paths by selecting the hardware piece as an output on a track, and recording it back into PT on a new track using the hardware as the input. That works just fine which tells me everything is connected right. I have a Lynx 16(n) interface and PT version 2018.1.0.

I have attached pictures of my I/O.

Humm I don't use a patch bay and I do use an Avid Carbon as my interface so consider that.

And If I am understanding you correctly
To use hardware as an "Insert" in PT , in the PT I/O The input and output have to be the same channel number.
Your I/O is showing radically different channels between the "Input" and "Output" tabs

For example you are showing your-- Tube Tech "Input" as channel 6 and it's "Output" as channel 3

On my system (again no patch bay but direct into and out of the Carbon interface )

So for example my two channel Bus Comp labeled (3U) = Input and Output are channels 5&6.
And My stereo HW Reverb - (M7) Inputs and Outputs on 7&8
And My "Insert tab" shows the same, 3U as 5&6 and M7 as 7&8

Input tab

Output tab

Are the patch bay circuits "normalled" correctly?

Its a Switchcraft Studio Patch 6425 64-point TT - DB25 Patchbay. I'm not sure what you mean by testing its continuity. I have made sure that the signal is running through the patchbay by using the hardware as the output on an audio track, and recording back into PT on another track with the hardware as the input.

The patchbay is full normalled. I really don't think its the patchbay; I suspect I have something messed up inside PT.
