Default ATTN: Mix hardware users - Scrub bug

If any Mix hardware/OS X users out there are experiencing a bug where when you scrub audio Pro Tools loses communication with the hardware and reports a DAE error 6039, PLEASE post your system information here in this thread and provide the following info:

1) What Mac are you using ?
2) Do you have an expansion chassis?
3) How many mix farms/DSP farms are you using? List all of them!
4) What version of Pro Tools are you using?
5) What version of MacOS X are you using?
6) What DAE buffer are you using (ie - 2, 4, 8)?
7) What is your CPU usage set to (in playback engine dialog)?
8) What is your hardware buffer set to (ie - 256/512/etc)?
9) Which audio interfaces are you using?
10) Do you have the "insertion follows scrub" preference turned on in Preferences:Operations: ?
11) What is the bit depth/sample rate/audio file type of a session where this bug occurs?
12) Can you provide an exact series of reproducible steps to cause the bug to occur?

Hi,

I scrub a lot (FX editor) - and while I've never had any problems I thought I'd post my specs to compare against (sorry, I hope this of use to you).

Dual 1GHz MDD G4
MIX Farm, d24, DSP Farm
PT 6.2.3 cs1
Mac OS X.3.2
DAE buffer 2
CPU usage 99%
256 sample buffer
882 (old 16 bit) interface
Insertion follows scrub is on
16/44.1/sd2 session type

No expansion chasis - and a 32 voice playback engine. You don't specify which you're having trouble with.

Could it be a bad card with you? Or are others having this trouble?
