Pro Tools 11 Hardware Insert Not Working

I'm not able to get a signal back into Pro Tools Vanilla 11.3.1 with UA Apollo Hardware. Is this because of the lack of input monitoring in PT11?

Everything is routed correctly - I've verified that signals are getting to the interface but they are not showing up in Pro Tools! I've tried the obvious enabling / disabling auto-input monitoring with no luck.

How do I enable input monitoring with this version of PT?
