There are 16 responses in the System Report form but none were HDX, and we were unable to repro in Pro Tools HD using midnightrambler's recipe.

Hey Jeffro - thanks for posting.
It is Soundworks UK who posted a couple of pages back twice about getting this on HDX. Maybe you can contact them? I see it is only once reported on HDX in this thread but nevertheless once is once too many to chance for me. Please keep us all posted on the progress towards the release of the fix... till then I will have to use other SW.

Is this issue known to affect systems with third-party soundcards?
I experienced several full-scale noise blasts earlier this week with HD 12.4 and RME UCX, but I can't really narrow down the cause because I was using several things in the session - RX4 with RX Monitor, Revoice Pro Monitor, auxes with delay and reverb. Edited to add: RX4, Revoice Pro 3, Maag EQ4, iZotope Alloy, Waves RVox, Waves H-Delay, Waves Abbey Road Plate, Softube Summit Grand Channel, Meldaproduction MAutoDynamicEQ, Avid Pro Limiter, Avid EQ3, Metric Halo Precision DeEsser, Meldaproduction MStereoSpread, Avid Dyn3. Thankfully I have a mute switch a couple inches from my console, but one of the blasts lasted for 5+ seconds when I was monitoring with headphones, and the mute switch only affects my monitors (they were muted at the time). My reaction was to throw the cans off my head and rip the plug from my interface's headphone jack. My hearing was shot for the rest of the day and it may have done damage to my cans as well, which is unfortunate because I don't think they make HFI-550 anymore

There is no cure for tinnitus or hearing damage. I got away with loud rock bands for 40+ years (first as drummer then E guitar and now mostly Acoustic Guitar) and only have minor Tinnitus despite ear protection (still awful though). I have spent the last 10 years carefully choosing gigs and doing tons of recording. Hearing is stable. One single noise blast (for me an incredibly sharp rimshot in an undamped room) can equal a permanent change in your hearing. Using a system that you know has this issue is a very interesting choice.
At the very least I hope you have a serious limiter for all of the monitoring lines... I am annoyed if something doesn't work right. Something that can cause real permanent damage to the user... that's a whole other thing.
