Selling from Canada to USA

Can anybody help with a FCC form 740 (fedex)? I am trying to ship 2 pci-x HD accel cards to the USA and it seems a bit complicated. I called fedex, searched Avid web site and browsed the internet without success.

I can't find the pci-x HD accel info for this:

1-Harmonized tariff number (also call HS code)

2-devide model #. (Could that be the FAB #? or the Assy#?)

3-the FFC ID
