Do you have a support case open for it or can you please open one. And let us here/Marianna know the case number. What Avid is after that would help are specifics like your system info, OS and Pro Tools versions.

This hit me twice or so in the past and I do not have any of that info that I can accurately describe this. It's folks are seeing it a lot there might be valuable clues from your setup or workflows, etc.

No support case yet. I wanted to crowdsource the symptoms a little here before I submit it. I’ll start one tomorrow. The real problem is that I’m not having the problem much any longer (since I rebuilt my template as I described). It was very evident in the past few years, but I don’t recall specific version numbers. Maybe I’ll try to break it this weekend.

Kerry what you are doing is great, but I was asking abt... it would be good that people who have specific problems with this can open support cases so Avid can get that info and be able to followup with them. Anybody where this happens more than the odd time in a few years (like it has for me, and where I cannot recall for sure Pro Tools setups/OS versions/VIs etc.) may well be very high value to get in touch with Avid. And Marianna has access to the right senior developers here to make progress but they need customer contact.

I’m still getting it. As someone mentioned earlier it’s usually when an instrument track is duplicated, but it’s inconsistent and very hard to repro, which is why i haven’t bothered with support case. (It also used to happen when you e.g. dragged auto-created MIDI files from a plug-in like Stylus to the edit window but they cleared that one up a few years ago.)

Gotcha. Go make your cases, people! . My case is made. Case #04769643 for Marianna or any other Customer Advocates
