Default MIDI crash - DEP Violation at: 0x0000000000000000

Let's get system specs out of the way first:

Pro Tools 12.4.0.294

I am trying to configure an M-Audio Keystation 49e for use with Pro Tools. It is connected via USB 2 cable. I have downloaded and installed the latest driver for this keyboard.

When I first get into PT, the following message appears:

"Pro Tools is unable to communicate with M-Audio Keyboard. Power-cycle M-Audio Keyboard and check it connections. Note, if M-Audio Keyboard is not in use, update the Peripherals dialog to prevent further warnings."

It is worth noting that the keyboard is connected, powered on and can be used in standalone software outside of Pro Tools: in this case, BFD3.
Device Manager reports all is well and the keyboard is properly identified and working.

Windows

I click "OK" to close the dialog.

I open SETUP > MIDI > MIDI INPUT DEVICES and USB Keystation 49e IN appears and has a check.

I open SETUP > PERIPHERALS > MIDI CONTROLLERS and there under #1 is
TYPE: "M-Audio Keybrd"
RECEIVE FROM: "USBKysttn49In"
SEND TO: "USBKysttn49Ot"

I change the SEND TO field to "BFD3 MIDI In 1" (since that is how I want to use it).

When I click OK, the following error appears:

"Access violation occurred, thread cause a DEP Violation at: 0x0000000000000000" and Pro Tools crashes.

I tried to attach the latest crash log but this site does not allow the extension.

Restarting Pro Tools and going back through the same process - any attempt to change SETUP > PERIPHERALS > MIDI CONTROLLERS results in the same error and crash.

Occasionally, I will get a bonus Windows error box:
"Microsoft Visual C++ Runtime Library
Runtime Error!
Program: C:\Program Files\Avid\Pro Tools\ProTools.exe
R6025 - pure virtual function call"

I trashed the contents of my USERS > APPDATA > ROAMING > AVID > PROTOOLS folder and restarted the system. No change.
I have also tried this with the Keystation disconnected from the system and with ti connected via MIDI cable through my interface: it makes no difference.

So... I have used this same M-Audio keyboard for years in earlier versions of Pro Tools under Windows XP and Windows 7. What can I do to get it properly configured?
