The fix for me was to update the drivers for my USB audio interface (PreSonus AudioBox 44VSL). I got Pro Tools to start acting right by unplugging everything USB, then installing ASIO4All and then...

I had this problem and it was pissing me off all day. The fix for me was to update the drivers for my USB interface. Mine is a PreSonus AudioBox 44VSL.
