I doubt we will see any more MBox2 updates, so you might be stuck with what you have.

The way to get it working with Yosemite is to first manually uninstall the latest version of the MBox2 drivers...

Yes, seems like a faulty channel on the AI16 card.

Also swapped the ribbon cable with the AI16 card below it in the MixRack, and the faulty channel moved across to 17/18.

It is using a workaround to do something very basic that should be easily performed withing the existing D-Command monitor section.
