I had a problem with the RME ADI-2 Pro fs refusing to connect to the S/PDIF output of the Carbon. Avid went away and fixed it with a firmware update.

No. Lots of people are fine.
But there have been problems with AU plugs and iLok in general and Carbon isn't certified for Sonoma.
So, you may be fine, but it's not guaranteed.

Try going back to 2023.12.1 perhaps.
Avid broke (removed) the legacy M-Audio midi driver in 2024.3. I suspect that’s what is wrong.

SOLVED: User error (as expected)!.
I was double routing to TotalMix, making DAW playback louder than QT playback.

I don't know how to run a LUFS meter against something being played in QT. It's the same file in the DAW as being played back in QT, so I'm either doing something stupid with Master Fader or post...

Good point, but HFS+ support is still there in the OS for legacy compatibility, it's not a good choice for a new system. APFS has been out for 7+ years now.
