It's odd that this just randomly started happening. It was working initially.

hold Option key > Go menu > Library > Preferences > Avid > Pro Tools > trash the Pro Tool Preferences file. Reboot your Mac, create a new PT session, and attempt your normal MIDI configuration.

This came from a support reply I finally got today!!!

Also, is the controller seen in the apple audio midi setup utility?
