Default Reason Rewire - How to route audio into separate PT tracks?

I'm using Reason 8 with Pro Tools 12 on Windows 10 and I'm having an issue getting different instruments in Reason to route back into Pro Tools into separate tracks.

Everything is syncing fine between the two programs, and PT is sending MIDI data into specific instruments in Reason just fine. The problem is the ouput audio from the Reason instruments are all being sent mixed together into each instrument track on PT. For example, I have four tracks, and if I solo any of them in PT, I get the audio from all four tracks coming through the single PT channel.

Is there something in the Reason mixer routing I need to change to get the audio to only be sent back to each corresponding PT track? Right now I can't apply plugins to individual tracks, and it's also very loud since each track is showing up in all four PT instrument channels.

In brief, assign each instrument in your Reason rack to it's own output in the Hardware Device module at the top to bypass the mixer:
