There's an old bug that just came up, it's the "insertion follows playback bug" where the insertion point after playback nudges back exactly 1 frame. This does not occur on fresh installations of protools 11 but is enables by changing your preferences to "insertion follows playback" in your preference file. This bug is REALLY annoying.

Is this happening with v11.2.1?

Yes, running 11.2.1 HD. The only thing that fixes the bug is deleting all preference files. I think what causes it is if you have under HD preferences "Timeline Insertion/Play Start Marker Follows Playback" checked, then, in the main bar uncheck "insertion follows playback" it'll start to do weird things. Also, if this doesn't make the bug appear try saving and quitting and then reloading the session repeating those steps.
