Default Avid Mbox Mini Mac driver bug glitch odd behavior

I am using the Avid Mbox Mini on a Mac Mini i7 with 16GB RAM & OS 10.7.5. I am using the latest Pro Tools Express 10.3.4 update, but the issues occurred prior to the update. Here's the system report.

When playing back audio from non-Pro Tools apps, I get some odd behavior from time to time. For example, in TwistedWave (my daily editor) occasionally I'll get a blast of noise for less than a second when I begin playback. It's very loud and hard on the ears.

Another issue is a loop of audio (<1 sec) will get "stuck" in what seems like the unit's internal buffer and keep playing until I unplug the USB cable. This can happen with just a Mac system sound, or during playback in TwistedWave.

Are these hardware, firmware, or driver related issues?

Does the issue occur on any other third party DAW?

I also have an issue that causes DAE 720 when starting playback that can only be fixed by unplugging and re-plugging the Mbox 3 Mini. Related?
This was happening before I when I was on OS 10.7.5, and the same exact issues continue now running OS 10.8.4 on PTE 10.3.4

Bump! Problem persists in Mac OS 10.8.4. NOT using a USB hub between the Mbox and Mac.
