*Pro Tools 7.1 , G5, HD3 crashes and issues

Ok, here is my problem. I have an HD3 with PCIe, 192 i/o, G5 Quad 2.5 Dual Processor, 2.5 gig RAM and a control 24. I have had my system set up for about a month and there are 4 issues.
1.Sometimes when I turn my G5 on my computer monitors don't appear. I have to restart my G5 for the monitors to come on.
2.Sometimes I get an error while in Pro Tools session saying that I need to shut down and I don't have any other option then to close out my session.
3.Sometimes during a corss fade the G5 locks up.
4.Then lately especially this past weekend and also AFTER I installed the Massive Pack 4. My PC shut down abruptly while listening to audio. This happened twice in my session. At the time the massive pack was not authorize on my ilok (but I doubt this would be the problem but if someone knows better please let me know).

I have the error report for both times my G5 crashed this weekend if anyone would like to take a shot at.

do a search in the duc! the crossfade thing is a know issue, fixed in pt7.1cs6! i think you will find solutions for the other probs! are your plugs in the compatibility list? which os, which pt software version etc, which plugins ....
