You're confusing Default EQ/DYN that are set in Prefs with plugin Favorites of which you can have as many as you want.

Favorites (described above) are rig specific.

Default Avid's plugin Menu organization is worse than...

Really, b/c I seem to remember it being only session specific.....they don't always show up at the top there

I guess I will have to dig into it again

This area was one of those things that Sonar had (maybe still has) that was aces - the ability to organize the plugins list however you wanted. Create folders and organize them how you want, name them what you want, etc. And it was global. You could also create multiple plugin lists so say if you only want to see certain ones during tracking - that's all you'd see. Mixing - create another list. And it was always possible to go back to the original listing that Sonar had.

Yes, stored in the prefs of the installation, not session file. Check it out.

The pref only displays the type that you entered in, not all plugs regardless of type etc
