Sounds like you may need to reinstall the OS. I believe "My extensions" is created automatically in the prefs folder, but Mac OS ext and Mac OS base ext are probably part of the OS itself.

Missing Mac Base Extensions
Does anyone have any suggestions to revive the Mac Base Extensions Manager. I've tried trashing the Extensions Preference folder and re-booting, but all I 've got are "My Extensions" ( I'm on a Mac OS 9)
