Default Clips turn into BEEEP

Hi, seasoned user here, but this bug baffles me.
I get a session send over by a client (Mac, PT2021.3.1) and some clips show only full blown waveforms and sound like a very (very!) loud beep. In the main session on her system, all plays nice, but it occurs after saving a copy, audio included. So already on her own system, something is not right.
The only solution up until now was to commit the troubled track and add that to the session. That seemed to work. But this morning I got a committed track that issued the same problem, but it occurred right in front of me: in this movie you can see it happen (sorry, no audio).

Trust me: just moments before it was the clip just before the one you see that had this problem.
Never seen it before: no plugins involved, whatsoever.
Any ideas what could cause this? Probably something header related right?
