Software Activation Code???

I'm about to send in the Transfer of Ownership form because I sold my PT vanilla license. The form asks for the "Software Activation Code" and I'm not sure where to find that. When I go to the "My Products" page and look at PT, I see a system ID number and also there's a code in the box labeled "User Notes." Is the Software Activation Code either one of those? If not, where can I find it?

It's the code that was used to redeem the product at my.avid.com, but you might not know that if redeeming it was handled by the purchase process. The form linked from the Transfer of Ownership KB says to include it if available, so it's okay to submit without that.
