"echoing/looping" of midi notes

Can someone point me in the right direction? with the following setup, I get a midi notes repeating like an echo while just playing my keyboard and monitoring the audio.
M-Audio Keystation Pro 88 midi out to midi in of 003
midi out of 003 to midi in of Novation Ultranova
Ultranova's SPDIF out to SPDIF in of 003
using a midi track for Pro 88 input and Ultranova output
using a stereo audio track for SPDIF input
I'm record-enabling these 2 tracks and playing Pro 88

when a note starts "looping/feeding back" I have to use "All Midi Notes OFF" command in PT to stop it.
I also have local turned off on the Ultranova (disables it's hardware keys/controls)
I've played around with buffer sizes/settings with no luck.

I did set system preferences and such as advised for optimization.

Do you possibly have the 88 connected via USB and MIDI cables at the same time?

Does the ultra behave properly if you simply use MIDI out on the 88 to MIDI in on the Ultra?

Does the ultra play back recorded MIDI correctly?

Thanks for the reply. I am powering the 88 via USB but did not allow AMS to recognize it as a USB device. In AMS, I created a new profile with only the 003 plugged in then added a device (Pro 88) as a generic device and routed its output to the 003 input (the way it is physically connected). The 88 does not show as a option for midi input but I guess it could somehow be doing something weird. I'll try powering the 88 via AC.

The Ultra does seem to behave properly when directly connected as long as local is turned off on the Ultra. If I turn on local and try simultaneously playing it's keys along with the 88's keys, notes will stick as if I were holding a sustain pedal. I have noticed a few glitchy issues with Ultranova so I can't rule it out.

The ultra does correctly playback midi recorded in PT.
