OMNI Mic Ch 2 Input Signal Issues

Before I have to shut down my studio for 4-6 weeks waiting on repairs, I'd like to ask if anyone else has had the same problem with their OMNI.
I've had mine since last Oct (2011), never a problem, but now after being gone on the road for 2 months, I've returned to a new problem. Mic IN channel 2 suddenly has little-to-no signal! And there is really no difference when I dial up the gain to the +65 max. Plugging in the back or front makes no difference, both the same result. Phantom power is ON and seems to be working properly.
What could be causing that? The preamp?

Other people have had problems with this. In most cases, reset the unit to factory specs (this is done with the setup button on the unit) will correct the problem.

I'm having the same problem. I've resetted... But no luck. Still, channel 2 doesn't receive phantom power.

Gotta get it fixed. Had my first OMNI fixed by Avid at no cost, then when it happened again they replaced it, no cost. Now this one has the same problem again and I just haven't gotten it in cause I've been busy. Using an external preamp that I already have to keep me going. It's a real drag.

They have a known issue with rev D and earlier versions. Ended up getting mine replace three times until they figured out the issue and then they gave me a rev E and it hasn't given me any trouble.

Contact Avid, they know about this and are pretty good about fixing it.

I have a Rev D with no problems, but my Rev F has both Mic Pre's with zero gain adjustment available...

I've had this issue for over a year and am looking into a new unit.

Can anyone comment on the difference between the board's "Board Rev" version and unit's "Rev" version.

For instance, listed on the serial number sticker on the box mine reads "Rev E" but when I access Setup>Ver>Board it reads "Rev A". Same serial on the box as the chassis.

Contact AVID Support. This is a known issue, and I had an Advance Replacement unit in my hands within days of posting this.

Was it within a year of the purchase? I'm out of warranty
