Default Random Errors in Sessions with Wrong delay on stereo pairs and auxes

I've been having an issue for quite a while now and its driving me nuts. I can't possibly be the only one who notices this stuff.

I'm on PTHD2 192 i/o 8x8 analog. V 9.0.5
Hexacore intel mac OS 10.6.8
Digitest passes everything fine, have trashed prefs/databases ect.

While I'm working on sessions (not the same session either, it happens randomly):

-I apply an audio suite plugin such as "gain" or "reverse" to a region of a track (note not RTAS or TDM, straight AS) All of a sudden output 4 is delayed by about 10-20ms. NOTHING i do can get it back, not cycling delay comp on/off, not anything. I can de-activate all my plugins, and nothing changes. or could be output 2 out of stereo pair 1/2. Seems to be random.
Sometimes its an aux send, pre or post fader... it will incorrectly delay one side by a BUNCH. Sometimes I instance a TDM, or rtas plugin, or something as simple as making a new track, and it'll do it. Only cure is a restart.

NOT COOL! I'm pulling my hair out because i can't seem to trust my PT rig with proper delay compensation anymore. Never quite sure, if my aux to reverb#2 is has one side slightly delayed. It's obvious to hear when its part of a stereo pair, such as 1/2. Here's the kicker... Its REALLY hard to hear and notice when one of your auxes send to your reverb is off...

-Also sometimes when tracking, I un-toggle delay compensation to BE OFF, to record some quick vocal OD's and don't really care if the reverb return has 1000 samples of latency, it's all good. But low and behold, the singer goes "my voice is delayed" And I'll re-cycle delay compensation on, then off again, and it'll seem to be ok. This has happened to me at MORE than my studio, it has been happening since version 8.xx at EVERY major studio I have worked at (Dozens).

Please tell me this is a known issue and being addressed. The random delay on sends or IO that are a stereo pair seems to be new to me once going to 9.0.5
The delay comp not turning of has been a nuance, but the shifting of one side of a stereo pair is driving me batty!

Just happened again... everything that came out of output pair 7/8 Left side was delayed around 2500 samples. I printed audio of it F-ed up, then did a simple re-start and printed it again, and all was fine.

What is going on... AVID? I will be calling monday.. This is not user error. I'm a PRO user, mixing albums daily.

I for a second thought my 192 was going out, but this is happening on internal routing busses... RANDOMLY! I just copied an Aux send to another track, and again some random Aux send (not the one I was copying) decided to delay one side by about 1000 samples.

I'm freaking out, its happening daily now. I even tried making a new session and importing the files/settings into it, in case my sessions were corrupt.

I usually work at 44.1 but this album has been at 48k thats the only thing thats diff, and NO it's not a clocking issue There's only 1x192 in the setup and its set to internal.

192 Voices 6DSP's 256 buffer / Playback, and cahe set to normal, and streaming buffer set to 2 (default) WTH is going on.

I have the exact same problem! I'm using the new Avid 16x16 IO though. Did you ever get to the bottom of it?
