Default NeoAccess Error

Can anyone offer any info on "Neo Access Error"

I'm using:
Pro Tools LE 6.4
Digi 001
Mac OSX: Panther (10.3.2)
Dual bootable Quicksilver G4 (I run PT LE 5.1 also)
RAM = 640mb (512 plus 128)
Two internal drives (about 120 gigs of storage; have about 90 gigs available)

Pro Tools LE 6.4 was running fine, but I started getting this "neoaccess error" last week when I launch the application. It launches half way, i see the error message, and the application closes.

I did notice that when I tried to open a 16 bit, 44.1Khz session from my desktop, the application did launch, but, when I tried to open the application again, it crashed.

Any suggestions?
