Default renaming regions/files

Hey guys. Does anyone know why:

when you double click a region in a session to change it's name, SOMETIMES it has "Change on Disk " option, but sometimes just "rename region"?? Would really like to know! Thanks guys.

When the region consists of the whole sound file, you get the option to rename the file as well.
