PT8 + 192 Digital I/O + Yamaha O2R

I am using Protools 8 HD1 with 192 Digital I/O, i am not able to get out from 9 to 16 channels but the I/O cards are good. digitest shows Loop Test failed from Channel 9 to 16. Core audio manager shows 8 in / 8 out, Slot information shows both I/O cards connected to Slot 1 & Slot 2 on clicking identify all 16 channel in & out LED glows. both I/O cards works individually on both slot 1 & 2 also when open 48 khz session audio gets distorted only with Yamaha o2r v2 ( works fine with other mixer)

unable to understand problem I am using ADAT Light Pipe cables, changed all flat cables & ADAT cables ( i don't have extra digilink cable to test )

please someone help me

Coreaudio will only show 8 i/o. Which device is the clock master?
How are you feeding clock to the O2r? Are you sure you have
selected Adat as the 192's second bank of digital outs?

Yamaha O2R is Master ( Word Clock) Using BNC Connectors

Yes i have selected ADAT as out

The O2R has ADAT cards? Have you tried to swap the cards and see if the signal follows?

i am not getting signal on 192 I/O itself (led does not glow) out selected properly in PT ( hardware setup shows 16in/out
