Default Input shows up 10dB+ hotter than actual input

Problem description:
I'm feeding the input (1-4) with tone, checking the input to make sure it's +4dB/1.228V. However, when I go to record, that tone is much hotter than what I had expected to see. Much hotter is 10dB+.

System description:
Powerbook G4, 1.5GHz, 2GB RAM, OSX 10.4.11, with Pro Tools 7.4. There's an external drive hooked up via Firewire 400. The 003 is set to +4 and the gain knob is turned all the way down.
