Default 1 Sample Gaps Bug OMF exports from Premiere

I've been receiving OMF files from an editor that has exactly 1 sample gaps in-between split/edit points with no crossfades. Doesn't occur at all the edit points just some, appears random but not 100% sure. Anybody else see this error? No idea what's causing this. I think it's coming from Adobe's side.

Using an old TDM HD system with ProTools 10HD on an even more antiquated MacPro. lol

Is drop-frame timecode being used? Sounds like a math rounding error.
