Default Error trying to create lost iLok case

I'm trying to submit a request via the dongle replacement form and getting an error message.
(Error trying to create lost iLok case. Please try again later or call the Customer Support.)
I'm trying to submit a request since aug 15th with no success
I have RMA number, lost iLok serial number, iLok user ID and ASC number.
Please help me resolve this issue.

Welcome to the community and sorry for the delay.

Your ASC is still in my.avid.com/products/notyetdownloaded so please click Activate Product then use the # it provides to submit your support case.
