Default 002R PT9 upgrade problems

I'm flumuxed. After running the PT9 upgrade (and driver) my 002R is acting really odd. I am NOT getting any errors/crashes in PT.

What is happening:
When I record enable a track the levels build until they peak (like feedback). I have found I can stop this by routing the output to somewhere else (like 3,4 if the input is 1,2). But if Input 1,2 goes to Output 1,2 then the screeching begins. Same is true if I route 3,4 to 3,4.

Also, once I do record (by routing output to 3,4) I cannot listen over headphones during record OR playback. Neither does the output reach my monitors (via the monitor out).

Now, when I open an older session and playback, all is well. I haven't yet tried recording in an old session because I really want to fix this.

I played around a little with the I/O settings and set them all to default. Also, playback engine all seems correct.

I'll add that I'm using TRS cables to route between 002 and a patchbay. I've used them before though and has always worked fine.

Any ideas gang? Something simple I'm missing?

Did you download the latest drivers for the 002? In PT9 and later, drivers are not installed with the Protools software, but are separately downloaded and installed.

thanks. right after i posted i installed the updated 9.01 driver.

then, after troubleshooting signal flow, swapping inputs, cables i gradually came to realize my patchbay was half-normaled!

Doh!
