First of all, you have to be running OS 9, because PT 5 won't run on OSX, not even in classic mode.

If you are in OS9, then I'd try trashing PT preferences files (I don't remember exactly where these are in OS9, but if memory serves you're looking for Pro Tools Prefs, DAE Prefs and DigiSetup). After you delete these, empty the trash and reboot. If that still doesn't do the trick, you might have to reinstall Pro Tools, and if that still doesn't work, you might have to reinstall the Mac OS.

Correct, and you have to choose Mac OS 9 as your Startup system. Pro Tools will not work in Classic when booted in OS X. From your previous post, it sounds like you may be trying to run Pro Tools in Classic:

Could be an extension conflict. Did you follow the guidelines for creating a new extension set for ProTools?

9.0.4 was not the best version of 9. You should update to 9.2.2 on a 400mhz G4.
