Default MIDI recorded latency after playing with no latency

Hi folks I'm new here and thank you in advance for any help on this!

Here's my setup:
Mac Pro (Late 2013), 3.5 GHz 6-Core, 16GB Memory, OSX 10.10.5 (Yosemite)
Universal Audio Apollo Quad w/ Thunderbolt (Console version 8.4.1)
Pro Tools LE 12.4.0
Akai MPK261 MIDI Keyboard (Using Pro Tools preset #9)

This never used to happen to me before PT 12, but I've noticed recently that when I record any MIDI track (this is with my buffer setting at 64), the actual MIDI notes themselves appear to be recording late (as if I was playing with my buffer setting at like 1024).

So for example, as I'm recording a MIDI cajon track as an Instrument Track with my buffer at 64, it sounds like I'm playing with minimal latency. But then when I playback, the notes are very noticeably behind what I THOUGHT I was playing. Another example, simple quarter notes that I feel like I'm playing right on the beat, are being recording slightly behind the beat on my grid.

I checked my MIDI settings and the AKAI website to make sure that my keyboard firmware/settings were solid, as well as my UA console software. Everything is up-to-date.

Please help. Also please excuse me if this is a ridiculously simple problem to fix!

can you do it at 32 sample? as always, it's a good habit to check both visually and aurally. you can always nudge or sync to transient. But do turn on the ADC.

Hey Hannibal thanks for your reply. I tried 32 and it's still exactly the same thing. I've been recording MIDI for a long time and before I started noticing this weird glitch, I used to be able to record even at 128 with no problems with the final recorded MIDI result. Delay comp is also engaged.

Re: nudging, I usually avoid having to nudge at all since I play most of my MIDI with the intent on not quantizing in the end.

I have no idea what is causing this...

Do you have any plugs on the Master? That will mess with delay compensation in a few scenarios.

Hey Chuck thanks for your reply. I actually don't use a master track or bus since I normally bounce my mix and then "master" the stereo file in a separate session.

Something I forgot to mention, all of my live audio is recording/playing back just fine, for instance, my live guitar or percussion tracks. It's only the MIDI that's acting weird!

There's actually another MIDI related issue that's been happening lately as well (which never used to happen before, since even the days of PT9), but I wanted to get to the bottom of this thing first.

Wierd like really Deja VU all over again.

Let me guess Kontakt with Cuba library?

I had the exact same issue- but I had a Apollo Quad with FireWire...

Same deal, played a rhythm part via midi and got way out of sync.....never figured it out, I was moving away from the Apollo anyway

No in this case I was using EastWest Storm Drum 3. But I also tried it with Kontakt and it's the exact same thing!

I don't think it's an Apollo thing because I like I said, this only started happening recently. I would venture to say that this has started happening in the last couple weeks.

Been recording MIDI with none of these issues for basically 5 years straight before this....ugh

At this point should I just buy an Avid support ticket and get it over with?

I noticed it around 12.2

Maybe a bit earlier than that actually, I can't remember exactly
