In my case, this has happened during a mix while a session was already open, though it hasn't happened often enough for me to figure out the exact conditions; it seems like it's occasionally after adding or deleting a track, but not sure.
For me, a reliable fix is to disable Eucon in Pro Tools and re-enable (don't need to close and open session). If it happens again I'll try and add an aux and see if that works.
