same problem here, PT12.8 on HDX with UAD Octo and UAD Quad cards.

in my case, it even shows up during playback, happens about 4 times a day when mixing, quite annoying!

my "solution" is to toggle the H/W Buffer Size (going back and forth between 256 and 512), then it goes back to normal.

I believe it is an issue with UAD integration.
