There were some versions, and I cannot remember which ones, where PT forgot the preset name of what you last used. It however kept the plugin settings intact, so please test if this is the case. Put a plugin into a channel, factory reset it, change one setting to extreme, save your settings to "test", last close/reopen and see if what I said holds true: the setting should be the extreme one (not factory default) but the preset name is forgotten.
