This problem often happens when you copy a session folder over to a new location and then open it in Pro Tools while the original is still present. If you don't move, rename or delete the original, Pro Tools thinks that the session allocation is still in the original location.

You can consolidate these sessions by doing a "Save Copy In" and include all audio/media files. This will bring everything in the session into a single session folder.
