Assuming you mean the HD/Native PCIe card? Yes it works with current software(using one on Windows 10 and the latest PT Ultimate here) and the 192 will still work as well. Here is what you may need:
1-Pro Tools HD or Ultimate software
2-If you go newer than 12.5HD software, you also need a Digilink license(for your iLok)
3-if you are using the old green key-shaped iLok, you need to get a newer gen 2 or gen 3 iLok(needed for 64 bit systems)
4-an adapter cable to connect the 192 digilink to the card's mini-digilink(often the adapter comes with the card, but do check)

Given that this is a major change to the system, if it were me, I would keep the old computer and TDM cards intact, in case you need to access something old. I would pick up a used Mac Pro(maybe bumping up to a 12 core with lots of RAM) and build a newer machine for all the newer software as many of your old plugins may be either changed, or not available in AAX64 format. Keeping the old machine in the closet could save a lot of hair pulling
