Indeed. Just went through the circus act of dragging all my RTAS plugs out of the folder and trying each one in turn, resetting prefs each time. Pluggo was the culprit for me too.

Only external devices can be "created". Not interfaces. These must be created by the driver, which apparently is not happening.

If you have a device that can generate timecode, you can simply record it to a track. Then send it out one of your analog outputs during playback. You don't need DV Toolkit just to do that.
