Hi folks, I've been running Pro Tools 2021.3 with my old 002Rack for the last 12 months, all ADAT and SPDIF ins and outs active - all's fine and dandy.

Thanks guys, I managed to solve it last night... only took 4 hours!

AMD Radeon Catalyst had installed a couple of random audio drivers, HDMI and SPDIF, and I had 2 onboard mobo sound drivers...
