I'm having the same issue, reproduction of the issue:

- drag stereo m4a audio file in region list
- drag audio file from region list into stereo track
- play the track
- while playing rename...

I have exaclty the same problem.
Almost every time I do this:
- goto the region list
- drag a file onto a track
- play the track
- in the region list rename the file (including remaning the file...
