I was able to fix the problem by uninstalling PT and all drivers. Then I reinstalled with the full installer and everything seems to be working fine.

I would strongly advise against Adobe Premiere, unless you want to edit the audio in Premiere. Very poor export options in Premiere, and no .omf support. You can't even export with a timecode burn....

It could be that the tapes you are chasing have time code drop outs in them. If it drops out at the same place everytime this is usually what the problem is.
