Try taking the Mac OS9 FireWire extensions back a version to v2.8.6 - this should work fine.

Try going back a version on the Mac OS9.X Fire Wire extensions - we we're having nightmares with these boxes and the new G4's - until we tried the older ones, once we did that all was tickety-boo!

If it's a dual channel card (EPCIDC) the connectors are called - 68 pin MD (micro density) or VHD (very high density.
