Yeah, we make it work using a system like that.

Related question: Why doesn't the click from Reaper start at zero? There is a slight delay before the click starts, which is why we use the audio...

Yeah, one would think that would solve the trick, but I've tried that with no luck. Maybe there are some bugs in my session. At least that behaviour doesn't apply when I export the MIDI. Any reason...

His session was created to a click, but the click of Reaper and Pro Tools doesn't line up properly. There is a delay at the beginning of the Reaper files. To avoid this issue I get his audio files...

You could try closing down PT, disconnecting and reconnecting the Mbox and restarting PT. There have been lots of problems with the Mbox 2 being silent after startup, but this worked for me.

I had the exact same problem when I was using in on a pc/Windows. After I switched to a MacBook Pro, it works fine (knock on wood).
