The quantization error is a signal unto itself. If you take all of the quantization error from every sample and you line them up you have a signal - a signal that is added to the original...

Truth is, if you recorded hot enough that you were teasing the red light then you probably DID clip even if all of the samples you recorded are "legal" (below full scale). One does not have to clip...

There's not a 100% linear way of doing it, but it can certainly be done to a degree that renders any resulting distortion inaudible.

Yes. Think of it like throwing a bunch of french fries on a cookie sheet and throwing them in the oven. They stick to the pan just a little bit as they cook. You pull it out and want to divvy them...

It's still a PLL in there. They might do something in there with a DSP chip (though I don't know what) but they still need a PLL in order to slave the internal clock to the outside.

The filter is less steep, but we use linear-phase filters in this industry for that task, now, so phase shift is not a problem.

Only with poorly implemented or non-linear EQs. If the EQ is linear (in other words not trying to resemble an analog EQ by adding some form of compression as well) there should be no reason for it...
