I never got to use my 30 day trial.

Is there any email form anywhere or a number that gets a quick response
from Avid?

Keep in mind that the trial license for Pro Tools is for an older version (Pro Tools 11)

There have been additions such as Track Freeze, Commit, and Collaboration, (to name a few) that appear in Pro Tools 12.

The best way to do this is to subscribe to Pro Tools for a month, and take advantage of the new features.
