Problems are most likely caused by you not following all the (complex) steps correctly, and other folks here cannot guess exactly what you have done.
Did you follow the Process for a lost iLok that was given earlier? What exactly have you done? Did you get responses from your RMA submission to iLok. Was the iLok synchronized or not... aka had you purchased iLok Zero Down Time? If not did you pay to create an Avid support case?

Pro Tools Perpetual Bundle (with 10/11/12) is the newer license bundle that added support for 12.6/higher and was deposited in your account in Sept 2016. It was never activated so it still needs your previous bundle to upgrade from (which you obviously can't do since the iLok containing that bundle is lost).

Found your support case and will respond to that, so please look for a case notice email...

Hi,

I lost my ilok back in 2015 and I submitted an RMA at the time ( I still have this ). I did not require Pro Tools at that time as I had changed careers and was working abroad. I have put together a small home studio for the first time in years and was hoping I could get my old Pro Tools 10 licence back.

I have submitted a case on avid using an old ACS code which seems to have went through. I always had a spare ilok and that is now synched to my ilok account and I am just running on PT first for the time being.

Can anyone confirm how long it will take to receive a response from Avid about this? and if I will even be able to obtain my old licence as the RMA was submitted back in 2015?
