AV option + HD = no luck

Anyone out there having any luck with the plain ole AV Option with HD? I spent three days, half of them on with tech support. No matter what we did, we couldn't get the video to load upon PT startup.
G4/PT 5.3.1/DIGI 13 SLOT CHASIS/AV OPTION.

Anyone having success?
