Default Reverb reduces volume of vocal

I have never had this happen before in Pro tools 9 but this happens all the time in Pro Tools 11. My vocal will be at normal level and then when I add reverb it will decrease the level by 5db.

Which reverb? How is routed? How is it addressed? How is it inserted?

If adding D-Verb please check the Volume (on the left) since it defaults to -4db.
